Registration Entitlements Full registration fee includes: • All Conference sessions Monday 19 September 2011 – Wednesday 21 September 2011 • Welcome Reception Monday 19 September 2011 • Morning/afternoon teas and lunches Monday 19 September 2011 – Wednesday 21 September 2011 • Satchel and ASM abstract book Day registration fee includes: • Conference sessions on one day Monday 19 September 2011 to Wednesday 21 September 2011 • Morning/afternoon tea and lunch on one day Monday 19 September 2011to Wednesday 21 September 2011 • Satchel and ASM abstract book The ASM Dinner Tuesday 20 September 2011 is an extra cost for all delegates Payment Registration forms must be accompanied by payment of the registration fees. Payment can be made in the following manner: Credit Card (Visa, MasterCard, AMEX) or Cheque made payable to: “ANZSN 2011 Conference” and mailed C/- Plevin and Associates Pty Ltd PO Box 54 BURNSIDE South Australia 5066 Cancellation Policy Should you be unable to attend a substitute delegate is welcome. All cancellations or alterations to registration must be made in writing to the Conference Secretariat – Plevin and Associates Pty Ltd. All registration forms will be received and accepted in good faith and processed as registered. Cancellations and ‘No Shows’ that have not paid will be charged the full registration fee and any incidentals including accommodation. Cancellation Date Refund Before 31/07/2011: A full refund less a cancellation fee of $150. All refunds will be paid after the conclusion of the Conference. Insurance Insurance, including medical cover and expenses incurred in the event the Conference is cancelled, is your responsibility. We recommend you discuss your insurance cover with your travel agent. The policy should include loss of fees/deposits through cancellation of your participation in the Conference or through the cancellation of the Conference itself, loss of airfares for any reason, medical expenses, loss or damage to personal property and additional expenses and repatriation should travel arrangements have to be altered. The Conference Secretariat will take no responsibility for any participant failing to insure. Disabled Facilities If you require disability specific facilities, please notify the secretariat by indicating on your registration form. Disclaimer In the event of industrial disruptions, the Conference Organisers cannot be held responsible for any losses incurred by delegates. The program is correct at the time of printing, but the Conference Organisers reserve the right to alter the program if and as is deemed necessary. Liability The Organising Committee and/or Conference Organiser shall not be held liable for personal accidents or losses or damage to private property of registered delegates of the Conference. Delegates should make their own arrangements with respect to personal insurance. |
