Personal information is collected on behalf of the Australian and New Zealand Society for Nephrology (ANZSN). The Commonwealth Privacy Act 1988 requires that you provide your consent to the inclusion of your personal information, such as name, employer and email, in the list of delegates for this Conference. This list will be made available to all other Conference delegates and Conference supporters/sponsors for the purpose of distributing related material. Members of the ANZSN Conference 2011 Conference Organising Committee will also receive copies of the list of delegates to enable the Committee to forward Conference information and materials to delegates if required. This list will also be provided to the organisers of the next ANZSN Conference scheduled for 2012. This list will not be provided by the ANZSN Conference Organising Committee to any other third party.
